We are growing and currently seeking a Clinical Navigator to join our Case Management team in New Berlin, WI! A hybrid work schedule can be arranged.
Summary
The Clinical Navigator is primarily responsible for the support and education of end-users, caregivers, and clinicians with Atos products and services in the Case Management Program. The Clinical Navigator works with the clinician in collaboration with the Atos Clinical Educators and Territory Sales Managers inquiries and concerns to assess, facilitate. plan and coordinate integrated product utilization for the end user. The navigator will develop and implement an individualized care plan in collaboration with the patient or patient’s support network to address the patient’s specific product needs and utilization to reach mutually established goals over time. The Navigator will work to ensure that the patient is making progress toward their desired outcomes.
Essential Functions
- Completes comprehensive assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member's health or psychosocial wellness, and triggers identified in the assessment.
- Develops and implements an individualized case management plan in collaboration with the patient, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals.
- Deliver evidence-based, enrollee-centered strategies and recommendations for situational use product utilization and conduct follow-up that facilitates the delivery of needed services to promote self-management.
- Partner with other entities involved in the member’s care, including medical providers, therapists, payor case management programs as well as community-based organization, in an effort to promote self-management and a high quality of life post laryngectomy.
- Performs ongoing monitoring of the care plan to evaluate effectiveness, document interventions, goal progression and achievement, product utilization and suggests changes accordingly to achieve patient’s desired outcome.
- Maintains ongoing member case load for regular outreach and management.
- Promotes utilization of products and services available for patients.
- Applies principles of self-management and motivational interviewing as well as Atos clinical guideposts to educate, support and motivate change during member contacts.
- Compile information and reports to produce metrics and overall case management program effectiveness and evaluation.
- Identify, create and present workflows within the case management program and interdisciplinary team to ensure optimum support in assisting patients with navigating the care delivery system.
- Assesses barriers to care, provides care coordination and assistance to members to address concerns.
Basic Qualifications
- Bachelor’s degree in healthcare related field required
- 4-5 years of direct patient care with laryngectomy or head and neck cancer experience required
- Current and/or extensive knowledge of pulmonary care required
Preferred Qualifications
- Experience caring for patients with laryngectomy in acute care setting preferred
