The Role -

• Provide professional and ethical care to patients.
• Support the patient journey from hospital to home through home visits and community-based clinics.
• Conduct virtual consultations via phone and video.
• Efficiently manage and organize your caseload and time.
• Communicate effectively with patients, carers, and referring healthcare professionals.
• Maintain clinical records in compliance with company, legal, and professional standards.
• Foster collaboration among various stakeholders, including acute and community healthcare teams.
• Build and maintain strong relationships with healthcare professionals across the region.
• Provide educational support to fellow healthcare professionals.
• Participate in service audits and evaluation activities.

Who are we looking for?

• Registered General Nurse.
• BSc Nursing degree or currently pursuing one.
• Stoma care experience is desirable but not essential.
• Strong interpersonal and relationship-building skills.
• Teaching experience.
• Proficiency in IT.
