We are looking for an experienced Nurse with relevant clinical skill and who has the passion, drive and commitment to deliver stoma and bowel care services within the Countess of Chester Hospital NHS Foundation Trust Stoma Team for 12 months.  Following this, there will be a mix of hospital and community work in the Chester area.

 

You will need to be able to work as an independent practitioner, be self-motivated and have a winning mindset. As well as being a strategical thinker who wants to build on their commercial awareness and skill.

The role

We are looking for a Specialist Nurse to work in Partnership with our NHS colleagues.  To provide ethical and professional nursing services within our Chronic Care Division (Bladder, Bowel and Stoma Care) within NMC guidelines.

•           Deliver professional and ethical care to patients

•           Provide specialist stoma and bowel care community clinic reviews

•           Support inpatient reviews/services

•           Support the patient pathway from hospital to home, providing home visits

•           Effectively and efficiently plan, manage and organise caseload and time

•           Communicate effectively and provide feedback to patients / carers and referring Healthcare professional

•           Maintain clinical records in accordance with company, legal and professional requirements

•           Instigate working practices to ensure collaborative working between different stakeholders e.g. acute and community health care teams

•           Generate and maintain effective working relationships with health care professionals across the region

•           Deliver educational support to fellow health care professionals in various settings

•           Undertake service audit/evaluation activities
