Project Description
Healthcare professionals (HCP) play an important role in ensuring that people with chronic conditions receive the right support beyond the clinical setting. However, transitions between hospital, community care and home can create gaps in education, follow-up, product access and continuity of support.
This project will explore how connected care services can create greater value for HCPs while supporting a smoother patient journey. Rather than focusing on marketing communication, the project should investigate how services, tools, workflows and collaboration models could address real needs in HCPs’ daily practice.
The students will identify relevant HCP needs, examine existing approaches and develop potential concepts that make it easier and more valuable for HCPs to connect suitable patients with continued support.
The final scope may focus on a specific market, HCP group, part of the patient journey or service concept, depending on the students’ interests and the agreed research direction.
The project is connected to a real strategic business challenge, while allowing students to apply relevant academic frameworks and shape the research according to their educational background.
Relevant Study Backgrounds
The project could draw on areas such as service design, customer experience, behavioural science, healthcare management, digital health, innovation or business strategy. The exact methodology will be agreed with the selected students.

• Which project(s) you are interested in – and why (maximum 2 projects per student/group)

• If you are applying with another student as a group, their full name
